OFFICIAL RULES ELIGIBILITY: NO PURCHASE NECESSARY TO WIN. Sweepstakes is open to legal residents of the 48 contiguous states, Alaska, Hawaii and the District of Columbia, who are 18 years or older at the time of entry. Employees of SparkPeople, Inc., Independent Health, the Buffalo Bills and affiliated entities and their immediate families (spouse, parents, children, siblings and their spouses) and individuals living in the same household as such employees are ineligible. Sweepstakes subject to all applicable federal, state, local laws and regulations. Winners may be required to complete relevant tax forms as a condition to the delivery of the applicable prize. HOW TO ENTER: The entry period is between October 6, 2014 and November 16, 2014. To enter: 1. Join the official "Independent Health Health & Wellness Challenge" by visiting http://www.buffalobills.com/fans/the-challenge.html?campaign=vanity_buf_page and entering your email address. 2. Log completion of daily fitness challenges by clicking the corresponding button for each daily challenge on the Challenge page over the 6-week period listed above. You will earn 5 points for logging each completed daily challenge. Each point automatically enters you into a drawing to win the prizes below. PRIZES: Odds of winning prizes depend on the total number of eligible entries received. Winners shall be responsible and liable for all federal, state and local taxes on the value of their prize. Please allow up to 6-8 weeks for prize delivery. Prizes will be fulfilled by the Buffalo Bills. One qualifying winner will be selected at random each week.
