I believe the book you are referring to is 33 Days to Morning Glory, by Fr. Michael Gaitley. I used it, along with St. Louis' book, to make my Consecration to the Immaculate Heart of Mary back in February, ending on March 25, 2013. It changed my life.
I would highly recommend the book! Both of them, although St. Louis' is a bit hard to understand. I had tried reading it for years, then Our Lady sent me Fr. Gaitley's book, and urged me to use both.

On Amazon I found this by doing a search on Morning Glory prayers. The book is written by an order priest (Marianists of the Immacuate Conception) and it is a distillation of the works of 4 saints or almost saints: Maximillian Kolbe, Louis deMontfort, Mother Teresa and John Paul II.

When all your desires are distilled, you will cast just two votes: to love more and to be happy.

I don't know about this particular form. I have used the deMontfort consecration. You read the book "True Devotion to Mary. Once you are through the readings you are supposed to go to confession and then for 3 weeks you say specific prayers along with your rosary. You should time this to make your consecration on either March 25 or April 28 as these are the feast days specified March is Our Lady and April is a feast day for St. Louis Marie de Montfort. You are supposed to renew your consecration each year an the anniversary, with preparation similar to the original.

I was wondering if anyone here has participated in or knows of this retreat. One of my coworkers is in the middle of it and I was reading a bit of her book.
Apparently it is a 6 week retreat where everyday you read about a page about a saint or their writings (Mother Theresa, St Maximilan Kobe are 2 examples) and once a week there is a discussion of the weeks reading at the church then at the end of the 6 weeks there is a ceremony and you consecrate yourself to our lady.
It sounds interesting to me but my friend doesn't know that much about the whole thing since she is only half way through.
