I have the same problem sort of. I do get motivated and do well then something happens in my house either someone is sick, stuff breaks, dogs are sick, kid has too many things to get to ect then I am not on top of my routines.
I do my best when I have a daily routine. Like I have stuff I do before work like house cleaning, planning what to have for dinner. Putting it in the crock pot if its a crock pot dinner. I go to my job then come home and I have things I do after work until bed time. Right now I am working on scheduling time to get to the gym a few times a week. My schedule tends to change to and that will throw me off as well.
A big help I found out is to plan a menu for each week. When I do this it makes things so much easier. I only cook one meal a day so its not hard. We are all out of the house during lunch so I dont worry about that. Same goes for breakfast. So I make sure we have dinner planned.
When life get a little nuts I just deal with what ever it is I have to deal with then when things settle down again I focus on myself again when it comes to getting healthy.
I am still reverting back to old habits that are easier to do sometimes. Not always good for me. The hard part is to get good habits like working out or walking into habits that you must do daily. Or making sure you have healthy options to eat in the kitchen so you are not tempted to eat any bad stuff. Its a life style change and I have come to realize this like losing weight does not happen over night. It takes time.
Hang in there you can do it. Just have to work on things a little at a time. :)
