Food logging and eating

I have not been consistent with logging my foods especially my evening meals no wonder I have not been losing weight. I go to the gym and lift weights 5 times a week. I do cardio three times a week--mind you I don't push myself on cardio days. No wonder I'm not seeing the results for which I wish.
So how do I change this?
I start logging everything I eat and follow a plan of eating to get me into shape instead of just supporting the out of control eating plan I am currently following. Uh!!
