Starting over. Again.

I have let myself coast too long. It's time to take the reins of my life and get control. Right now I'm prediabetic, my cholesterol is crazy, and I have this week hit my highest weight ever. I want more than anything to be a mom, and having a healthy pregnancy and healthy kids starts with being a healthy me.
I want to be my best self, and I'm ready to take the steps to reach that goal. So, I have small steps I'm planning to implement. To start off with, I'm tracking my food all day, every day. I will then slowly, but surely, start making changes in my food choices.
I am currently making my plans for exercise. They're not set in stone, but my basic idea right now is: Yoga and/or deep stretching at least 5 days a week; Cardio (working up to longer times, but starting with "whatever I can do") at least 3 days a week; Strength training at least 3 days a week.
My short term goals: Be down to 230 pounds my mid-September 2016; Have Triglycerides down to 199 by January 2017; Have A-1C down to 5.6% by January 2017.
My long term goals: Get back into theatre; Have a healthy pregnancy (and then another one, and another one, and...); Get down to about 115 pounds; Get my triglycerides into health range (under 150).
My current fitness goals: Get upper body strength up to being able to do a traditional push-up; Be able to walk a mile on the treadmill without holding the handrails.
What I'm working towards nutrition-wise: No more than 1650 calories at least 5 days a week; Never above 2000 calories; More fruits, veggies, chicken, and fish; At least 48 ounces of water a day.
I'm going to get up to these steps and goals slowly and realistically. This is a life change, not a short-term weight loss solution.
