Accountability & Good Habits

In my effort to restart after a lax performance last month, I came up with this chart to help keep me accountable. I think it will help on a daily basis to see how much I'm doing (not doing!) to instill good habits.
I tried to frame tasks in positive / proactive language and to pinpoint problem areas (eating too many high carb snacks, indulging in mints and candy too often, browsing facebook (even tho I think it is not a great place for me, mentally) before doing what needs to be done, staying within calorie limits).
I do like that it isn't perfection based (to trick myself into not giving up if I don't do everything, always). I also left room for more good habits - any ideas?
