I got your post on my blog page about hypothyroidism. You should really slap your doctor around and tell him you demand the test and if your doctor still won't test it I would get a different doctor. Hypothyroidism is real and not just an "excuse" to be fat. Thyroid levels also fluctuate each day you need to find a doctor who understands this because your first test may come back "normal" Always ask for a copy of your tests too. Your goal is to get your TSH levels around 1. As for caffeine etc B vitamins are helping, caffiene I try to limit but that is hard because I work nights, avoiding soy and carefully monitoring Sodium keep it in range toward the lower end. I am also trying CLA which seems to be helping with lean muscle. I recommend joining Thyroid Community and Checking out the book The Thyroid Diet by Mary Shomon it should give you a leg to stand on when talking to your doctor. Sorry this is so long.
