When I get home from the store, as I'm putting my groceries away, I cut up fruit and vegetables into portions and put them in bags so when I'm hungry, all I have to do is open the drawer and grab a bag, rather than grazing my pantry for something I really shouldn't have.

I like to pre-measure snacks like pretzels, nuts, chips etc. I make sure they are around 100 cal. and then put them in snack baggies. Makes it a lot easier to just grab a snack and know that I'm not mindlessly eating.
If you like chips, then figure out what you can substitute to "get your fix." For chips, perhaps Kashi Cinnamon Cereal in a pre-measured snack bags...ready to grab. Popcorn is another good alternative (100 calorie bags).
Keep something healthy and convenient at the front of your fridge so you can grab it quickly when you need something NOW. Whether you pre-package your own portions or use grocery store convenience foods like carrot snack packs, it will make a huge difference in what you eat if it's something easy for you to grab.
You can pre-portion salad into bowls, pre-cut vegetables for snacking, and pre-portion nuts so you don't overdo it.
