Find numerous places to workout and find activities you really enjoy
I learned to workout at home after always working at the gym. I found DVDs that I really enjoy like a Bollywood workout and Zumba and it's easier to throw on workout clothes than driving to the gym. I also take a break at work at lunch to walk on the treadmill. And then there's always the gym too.
Having some many possibilities for working out gives me no excuses not to do it. And I've also found things I really enjoy and look forward to.
When we go out to a restaurant I always consult the nutrition plan of the restaurant or the "calorie king" book. Depending on the calorie count I divide my portions & request for a to go box before I start eating. It always works!!! Most places offer healthy food substitutes. Always plan ahead!!
Track fitness AND nutrition the night before or in the AM!
This always works for me. Always. If I log my entire day nutritionally whenever possible, and if I at least log WHAT I'm going to do fitness wise (but maybe unsure of how many minutes it'll take), it motivates me to eat right and do it. Deleting an exercise bc you didn't do it or adding a food to make you go over your calories for the day stinks, believe me! :)
