Really ask yourself: am I distracted during a workout? Am I snacking when I walk in the door? Am I picking at leftovers after dinner? Am I managing my stress?
Sometimes you have to end the delusion by writing down your food very accurately for the day. You know, there's cleaning the kitchen and then there's REALLY cleaning the kitchen.
