Sign up to a gym/fitness centre near your place of work. Get changed at work if you can so there is no excuse of going home to get changed and then felling like you can't be bothered to go back out again.

I invest some time once a week to plan meals for the week ahead. Then the grocery list will be done, and when I get home from the store, I immediately enter any new food items I purchased into my favourites...that way I don't have to worry about it later or when I'm away from home and use my iPhone App. I also bag everything into 1 servings or into containers from Fit N Fresh. I love them!!! It's SOOO worth it to take some time to get it sorted.
Everytime I make an excuse to not be healthy and active. I write the excuse down and have a no excuse list. I post it so I can see how many times I try to weasel out. Example: excuse-can't afford gym well could I afford monthly rx because of bad health?
Whenever I find myself making excuses for why I can't eat healthy today or why I can't go to the gym, I stop and think them through instead of just agreeing with them. It's like having a debate. I ask myself if the excuse is true and if it's worth the setback. As long as I force myself to face the excuse, I find myself eating well and heading to the gym. The excuse is not worth it.
