Greetings from the SparkPeople Public Relations Team! My name is Terri Vian and I work with Bruce Corwin, SparkPeople Communications Director. Part of our role is to promote and celebrate success stories like yours on the SparkPeople site as well as in local and national media sources. We are reaching out to the members of the At Goal and Maintaining + Transitioning to Maintenance Team, specifically, because of your proven success and achievements! If you are willing, we would really appreciate it if you would complete our Official Success Story Questionnaire. To do so, please follow this link:

docs.google.com/spreadsheet/viewform?fromE
mail=true&formkey=dGw3SlJTbVU0X3U2WkN1
XzJLdWVBQlE6MQ

Additionally, we’d like to ask you to email us “before” and “after” photos to go along with your questionnaire. If possible, please send full body shots (2 each, before and after, if you have them) to SparkPeopleSuccess@gmail.com using the subject line: “Success Story Photo Submission”. You may send photos already on your SparkPage or ask a friend or family member to help you take some new ‘after’ shots- whatever you feel would show you in the most flattering light!

What happens after we receive your materials? Well, your story and photos are added to our 'Success Story Database' (yes, there is such a thing) and when we receive specific requests from the SP editors or external media sources (magazines, TV, etc.), we have all of your materials ready to go! Don't worry, we always contact you prior to any submissions, so you have the final choice in how your information is used.

Congratulations on your health and wellness achievements- we would love the opportunity to feature your story as a motivating example for others! Thank you in advance for your time and consideration. If you have any questions, please feel free to contact me via email at SparkPeopleSuccess@gmail.com or via SparkMail (SPTerriV).

SparkCheers,
Terri Vian
