Zach was not truly back. It was a flashback on the case they worked when they first met, which was a year (TVland time) before we met them in the pilot episode. So Zach would have still been with Bones as her grad student.
We already knew that Cam and Booth have known each other for quite some time and we knew that they dated before, but we don't know exactly when. So them knowing each other but not dating a year before the pilot case is believable. We also already knew that Cam was the NYC Medical Examiner and she was hired while Bones was on vacation. Dr. Goodman purposely didn't tell Brennan. So they did go over her head. That was part of the issue Bones had with Cam in season 2.
I loved this episode. At first I was devastated by then ending, but I agree with the person who said that Booth planted a seed. I think that is why he backed down so fast and said that she was right. She confirmed that she feels the same way for him, but her head is telling her not to give them a chance because she's afraid she'll hurt him. Now that it's all out there in the open, I think they'll start progressing more. They won't be able to just pretend like that didn't happen and they didn't have that conversation.
Also, David Boreanaz said in an interview that he kind of courts Bones this season. I haven't seen any courting so far, so I think last night's episode was the turning point.
