That would be hard to do without evaluating the person first. There are 20-30 causes of neck pain that you could be having from an injury 4 years ago. Giving you exercise over the internet could end up hurting you worse.
I suggest you find a good therapist in your area. If you need help doing that let me know. APTA.org has a place on the website where you can plug in your zip code and find PT's in your area. If no one is on that list nearby we can contact your state chapter of the APTA (American Physical Therapy Association) and find a good therapist in your area.
