So sorry you're having problems with this. Sounds horrible.
I get nite time leg cramps. There's several factors that trigger them.
I'm not you but perhaps sharing some of my stuff will help?
I won't go into the long and lengthy list... but the one that may apply to your circumstance (morning, before food, ie a fasting state) is low blood sugar... hypoglycemia.
There's an easy, in expensive way to test this.
In the diabetic section of any drug store, are glucose pills.
They cost about $1-$2. My fav is the raspberry.
Just keep them by your beside, and when the cramps start, pop a pill.
If your cramps disappear after 10 -20 minutes, then there's a good chance hypoglycemia is the cause.
Alternately you can purchase a glucose meter, but that's really pricey.
AND it involves pricking you finger, which when you're writhing in agony, is prolly not even do-able.
Hypoglycemia can be caused by things other than diabetes.
Often its because of adrenal insufficiency that hasn't progressed to full blown Addison's Disease. It can either be caused with a problem in the adrenals themselves, or with the pituitary or hypothalamus.
Has your doctor checked your electrolytes (sodium, potassium, calcium, potassium)?
The beginnings of adrenal problems will show hyponatremia... low sodium... and high potassium. If your calcium is elevated, it may be your parathyroid gland.
In any cause, I'd try the glucose pills first.

Another thing... some of us (including myself) on my thyroid forum are taking a form of silica.
Either DE... Diatomaceous Earth... or Orthosilicic Acid (Biosil or Jarrosil).
We've found less incidence of leg cramping using these... it was an unexpected side bennie.
DE is also good for improving your LDL cholesterol profile, by dropping it about 20 points, building collagen, which is great for skin, connective tissue and joints. Including the lining of the arteries, making them more flexible.
In any case, I sure hope you find something that works for you.

I've suffered horribly from this... literally going for months on end with little or no sleep because every time I slipped off, I'd cramp up. I've also gotten them in my inner thighs, its impossible to get out of bed, because both legs are in such a spasm that I'd fall and not be able to get up.
Made me cranky!
