I can see where I can get a lot of information here in dealing with my husband renal failure leading to dialysis soon. I'm not in renal failure but my renal functions aren normal and my own GRF is abnormal. Some medications I quit taking recendtly seem to be helping.

The workshop mentioned here was conducted at the Franciscan Medical Group's St. Joseph hospital in Tacoma WA. It was free, though a doctor's referral was required--but you could bring a guest/caregiver.

Most major medical centers and/or dialysis centers have similar programs available. They are usually conducted by a team consisting of a medical practioner and a renal nutrition specialist. You can also order materials (again, usually free) or check for upcoming events with the American Association of Kidney Patients online at www.aakp.org/

You can join even though you don't use NxStage but are a caretaker/dialysis patient/future patient. I'm sure every one has read this, but here it goes again.
Grape fruit Juice, can interfere with some of the Medicine you take. Also doing research and asking my Nephrologist, Orange Juice does the same thing. Apple Juice and Grape Juice can interfere with some antibiotics.
I always take my medicine with water, i hope you do too. emoticon

All good info on specialized diets for CKD. Advise for seeing a RENAL dietician is great! My renal dietician recommended a book to read called, "The China Study", which opened my eyes to nutrition in general. If you can, get a copy of it and see the results of numerous studies around the world of how nutrition impacts disease - - both positively and negatively. Keep tracking those sodium, potassium and phosphorus levels!

I hadn't heard of this, so I went looking.

It's true.

Star fruit apparently contains a neurotoxin that healthy kidneys usually filter out. If you have renal insufficiency for any reason, you put yourself in jeopardy eating them: if you do not receive DAILY CONTINUOUS HAEMODIALYSIS until all trace of the neurotoxin is gone, this 'star fruit intoxication' is fatal. Peritoneal dialysis apparently isn't enough.

Here's one of the articles I found. There were dozens, I was very surprised!

I saw this on a kidney article the other day.
AVOID STARFRUIT-----Its hazardous for Dialysis Patients, it is known to cause Agitation, Confusion, and possible death.
Has any one else heard of this? Im not on dialysis but i believe i would avoid this fruit now, with any stage of CKD.

Too many foods today for them to include them all, I suppose...LOL

I knew about the watermelon, there are a lot of fruits that contain higher amounts of potassium than you'd expect--not to mention the odd vegetables--and of course, animal products have ALWAYS had a lot in them! (not to mention phosphorus!) I had no clue, though, about the arugula--thanks for the heads-up!...

I always wondered why I didn't really like the taste of that stuff... now I have an even BETTER reason not to buy it! LOL

Watermelon is high in potassium and so is Arugula. I almost didn't get my transplant because I had a salad with Arugula in it the night befor. I had to take some medicine to flush it out of my system as I didn't have an access for dyalisis. And don' t forget carrots have protien when figuring your daily totals.

I am more than happy to share the little bit I know, as there are many who don't have access to classes etc... I did notice that the only thing I know for sure is really new is the 1500 mg sodium limit... A 'Medical News Today' article had this to say:

" "The 2010 Dietary Guidelines are being released at a time when the majority of adults and one in three children is overweight or obese and this is a crisis we can no longer ignore. These new and improved dietary recommendations give individuals the information to make thoughtful choices of healthier foods in the right portions and to complement those choices with physical activity. The bottom line is that most Americans need to trim our waistlines to reduce the risk of development diet-related chronic disease. Improving our eating habits is not only good for every individuals and family, but also for our country."


The new guidelines include 23 specific recommendations for the general population and six recommendations for specific groups such as pregnant women.

No one should consume more than 2,300 milligrams of salt per day. Those who are age 51 and older and those who are African American (almost half of the U.S. population) or have high blood pressure, diabetes or chronic kidney disease should consume no more than 1,500 milligrams per day.

Kidneys naturally balance the amount of sodium stored in the body for optimal health. When sodium levels are low, kidneys essentially hold on to the sodium. When sodium levels are high, kidneys excrete the excess in urine.

However if for some reason the kidneys can't eliminate enough sodium, the sodium starts to accumulate in the bloodstream. Because sodium attracts and holds water, blood volume increases. Increased blood volume makes the heart work harder to move more blood through blood vessels, which increases pressure in the arteries. Such diseases as congestive heart failure, cirrhosis and chronic kidney disease can make it hard for kidneys to keep sodium levels balanced.

Just a pinch of salt here and there can quickly add up to unhealthy levels of sodium. One teaspoon of table salt has 2,325 milligrams (mg) of sodium. Many processed and prepared foods already contain lots of sodium, be aware that not just adding table salt to your baked potato is dangerous.

Everyone should consume less than 10 percent of their calories from saturated fats by replacing them with monounsaturated and polyunsaturated fats, and intake less than 300 milligrams of cholesterol each day..."

I use a lot of lemon as I think I mentioned. Also, vinegar or vinaigrette dressings... they work just as well on cooked veggies and as a marinade for meats as they do for salads. I discovered too that there is a salt free chicken bouillon too, but it has potassium chloride in it so... I tried it once--yuck! LOL I haven't read the labels on the Dash products yet, but the ones I tried in the early days did not appeal to my tastebuds at all... I mix my own from dried herbs and seasonings or go without.

...it really is amazing, the flavor of natural organically grown foods...once you get used to what they are supposed to taste like without the preservatives. That's actually one of salt's primary uses... don't know if any one knew this but salt used to be used as currency. Used to be it was hard to come by, and everyone needed it, so... The word 'salary' comes from salt, and it was used to pay people for their services. It's so cheap and easy to come by now, I don't think I'd want to be paid in sodium chloride! LOL

Kathy i use a lot of lemon instead of salt. Also Mrs Dash, salt free seasoning, Braggs,salt free seasonings. I use spices, its like herbs,CKD patients cant use a lot of these.
Kathy do you have use to a Fax machine, i have one and i would love to have all this information you have from the classes you are taking.

I know you can buy salt free Tuna, also baking powder with out phosphorus, i personally try to watch for the high glucose sugar thing also.

Just read through that, there are a few things we talked about that aren't in it: NO 'light' salts, they are primarily potassium chloride.

You can use apple cider vinegar or lemon juice with dill to add flavor to vegetables instead of using salt...

Add your own suggestions too, it's definitely appropriate here!

NUTRITION FOR PEOPLE WITH CHRONIC KIDNEY DISEASE
REGULAR TESTING FOR POTASSIUM, CALCIUM, PHOSPHORUS, VITAMIN D AND B12 ARE RECOMMENDED. MOST NEPHROLOGISTS OR DOCTORS WILL KNOW TO DO THIS IF YOU HAVE CKD.
POTASSIUM
Control Your Potassium Intake.

Potassium is a mineral found in many fruits and vegetables, and it helps our nerves, muscles, and heart work properly. For people with CKD, the kidneys can no longer remove excess potassium, so levels can build up in the blood, leading to nausea, weakness, irregular heartbeat, and even heart failure. So it’s important to control the amount of high-potassium foods you eat.

GOOD CHOICES
Apples, berries, grapes, peaches, plums, pineapple, watermelon
Cranberry juice, apple juice, grape juice (a single serving is FOUR ounces. not six, not eight)
Carrots, green beans cauliflower, eggplant cucumbers, lettuce, mushrooms
Non-dairy whipped topping, non-dairy creamer, sherbet, sorbet
Unenriched rice milk, non-dairy creamer

POOR CHOICES
Bananas, oranges, avocado, cantaloupe, honeydew, apricots, nectarines, papaya, kiwi, mangoes
Tomatoes, spinach, potatoes, greens, pumpkin
Nuts, cooked dried beans, peas, seeds
Yogurt, ice cream, milk

SODIUM IN YOUR DIET

LIMIT THESE HIGHLY SALTED, HIGH-SODIUM FOODS
Fast foods: Pizza, Chinese, Mexican, and deluxe hamburger and fries
Condiments: Salt, garlic salt, onion salt, celery salt, souy sauce, steak and barbeque sauce, teriyaki and Worcestershire sauce
Snack foods: All salted chips, pretzels, crackers and pork rinds
Soups: Instant, canned, including broth and bouillon
Sauces: Canned or jarred sauces and gravy, instant sauce and gravy mixes
Meats: Smoked, salted or cured meats such as bacon, ham, hot dogs, sausage, bologna, salami, pepperoni luncheon meats, corned beef, pastrami and chipped beef
Dairy: Processed cheese, cheese spread and buttermilk
Seafood: Canned tuna, sardines, anchovies, crab oysters
Vegetables: Canned salt, frozen vegetables in sauces, olives, pickles and sauerkraut
Pasta and rice: Boxed and flavored mixes of rice, pasta, instant noodles and stuffing

CHOOSE FRESH FOODS
Fresh foods have no added salt, unlike canned and processed foods.

READ FOOD LABELS.
Choose snacks and side dishes less than 150mg of sodium per serving
Choose full meals or entrees less than 600mg of sodium.
Always look at the serving size.
Read the ingredient list. If salt or an ingredient listing the word “sodium” is in the first three or four items, avoid that product.

AVOID SALT AT THE TABLE.
At home, remove the salt shaker from the table.
At restaurants, use caution ordering food with sauces and condiments. Ask that salt not be used in preparing your food.
Spice your food with black pepper, hot pepper sauce, crushed red pepper, garlic or onion powder.

SEASON WITH HERBS
Alspice: ground meats, stews, salads, soups, sauces
Basil: eggs, fish, lamb, beef, stews, salads, soups, sauces
Bay leaves: meats, stews, chicken, turkey, soups
Chives: eggs, salads, sauces, soups, meats, vegetables, rice
Cider vinegar: salads, vegetables, sauces and marinades
Cilantro: meats, chicken, fish, rice, pasta and beans
Cinnamon: fruits, bread an dhot cereals
Curry powder: meats, chicken, fish
Dill weed: fish, soups, Pasta
Garlic: meats poultry, fish, vegetables, ric and pasta
Ginger: chicken, fish, fruits
Mustard powder: ground meats, salads, sauces
Paprika: meats, fish, stews, sauces, soups, vegetables
Rosemary: chicken, veal, meats, bread, pasta
Sage: meats, stews, green beans
Thyme: eggs, meats, sauces, soups, vegetables



PHOSPHORUS

Stage II and higher benefit from a low phosphorus diet. Benefits can be seen even when blood values are normal.

HIGH PHOSPHORUS FOODS
Milk, Cheese, Ice Cream, Pudding/custards, Yougurt, Coottage Cheese, Macaroni & Cheese, Pizza, Chicken Nuggets, Nuts, Bran Cereals/muffins

HIDDEN SOURCES OF PHOSPHORUS
Processed foods, Dark colas, Baked goods
READ LABELS FOR ADDED PHOSPHORUS (Anything where you see the word phosphate)
ALSO WATCH FOR PACKAGING MATERIALS—cardboard, and some plastics like those juices or flavored waters come in have phosphate added as a preservative, which can leach into the contents.

BUTTER, MARGARINE, SOUR CREAM AND CREAM CHEESE ARE LOW IN PHOSPHORUS AND OKAY FOR CONSUMPTION

PROTEIN

Most patients with Stage II or higher benefit from a low protein diet. Stages III-IV need careful planning. Caution is necessary if you are losing weight, especially if poor appetite becomes a problem.
Use more plant based proteins such as tofu, beans (High potassium, so no more than ˝ cup per day) and meat analogs (saitan, gluten, tvp.)

Limit red meat and pork
Use more fish, poultry and egg whites
Add one soy product per day i.e. soy milk, tofu or veggie burgers
Keep protein foods to 2 small (3oz) servings per day

DIETARY NEEDS FOR THOSE ON DIALYSIS ARE DIFFERENT. THEY DO NEED TO RESTRICT THE NORMAL THINGS LIKE SODIUM, POTASSIUM AND PHOSPHORUS—BUT REQUIRE LOW FLUIDS AS WELL—AND HIGH PROTEIN—SO IF ON DIALYSIS YOU WILL NEED TO CONSULT WITH A PROFESSIONAL TO HELP YOU DETERMINE THE RIGHT LEVELS FOR YOU.


STAYING HYDRATED IS IMPORTANT, FLUID RESTRICTIONS USUALLY ONLY APPLY TO STAGE V WHEN DIALYSIS TREATMENTS ARE REQUIRED. YOUR NEPHROLOGIST OR OTHER KIDNEY SPECIALIST WILL INFORM YOU IF AND WHEN THIS BECOMES NECESSARY.

SYMPTOMS OF KIDNEY PROBLEMS/UREMIA

Poor taste. Metallic, bitter or “off”.
Itching skin
Upset stomach or nausea
Unexplained weight loss
Headache
Weakness/fatigue

If unexplained weight loss occurs it may become necessary to use supplements such as Suplena, Benecalorie, Boost, Novasource Renal…

VITAMINS AND MINERAL
Stage III and lower “multi-vitamin”
Stage IV and higher renal vitamin
AVOID all other minerals and herbs unless you check with a renal dietitian, nephrologist or renal pharmacist.

Linda i stress diet, so much, as to what i can have and cant, by what my Dr. said after blood work. Each time i have blood work it seems one thing will be good and another i have to work on. My dog likes to take me for walks, she loves to get on her leash and walk.

I'm like you i do believe in miracles, changing your diet may help you, and i do so stress a Renal Dietitian if you haven't been to one. I would think your Dr. would have sent you to one by now.. Notice i said Renal Dietitian not just a dietitian, Renal knows all about kidneys, the others most of the time dont know about the kidneys as much. We are here for you. Are you close to me? I'm in Knoxville or close to it.

Hi Caroline. Yes, I am in stage 5 renal failure. However, I do pretty well! My mineral levels are all perfect. Vit D3 is 54. It is my rreatinine of 3.8 and BUN of 87 that are out of range. I am spilling protein and blood in my urine. What happened was I had undiagnosed hyperparathyroidism for at least 10 years and that is what shut down my kidneys. My nephrologist says they are scarred beyond repair. However, I believe in miracles. I think they happen every day. And even if I end up on dialysis, I would expect the miracle of peace and happiness to permeate my life. I am very happy to have met you and the others in this group. I just don't feel so alone. Thank you!

Do you have a Nephrologist? When they do blood work, it will tell you what you are high in and low in as far as Sodium,Phosphorus,Potassium, Protein, Calcium, Iron, Vitamin D. Then you will know more of what you can and cannot eat. I stress DIET,DIET,DIET food you can and can't have is what i mean by diet. And Jeanla and i both follow the Renal Diet. Its is strict, but you can bring your GFR up. Did you know there are 5 stages of Kidney Disease? If your GFR is 13 i dont know how you are still going, Less than 15 is Kidney Failure. Tell me a little more about your self and what you do know, or what the Dr. has told you.

Linda i know you will enjoy the booklets, lots of good information. Wow your GFR is very low, when mine dropped to 29 i was in panic mode. I got busy and brought it up to 38 which is still low, im still working on i though. Never give up.

I got in touch with the AAKP today and ordered all their brochures! They said no problem and would get them right out to me. Here's hoping I can use this information to improve my GFR! It's 13 right now. If that doesn't happen, at least I will know I have done all I can.

CKD IS a little scary! LOL I'm hoping there will be some fun things down the line... who knows? You find joke books in a library too, right? Though we probably shouldn't overdo it-- we might make too much noise, huh. LOL
