PMS will definitely do it. I find I often mindlessly eat more during that time of the month.
I also found when I don't track, I think I've eaten far less than I actually have. It took me a week of just focusing on the tracker to learn how to use it most effectively, and how to quickly add things I eat that were not there, but it was so worth it.
I do have to admit, I am not a weigher of my foods. I will until I am able to look at it and judge how much is there. I still tend to just judge more often than not.
Hope you are able to work out this and figure out why the scale is going up instead of down.
