! Lowering sodium and watching your sugars is possible. In fact, since you have some similar issues, I would like to also invite you to take a look at the Living Healthy w/ Insulin Resistance / Low GI Diet team.
Having to watch your sodium can be difficult, though through in having to watch sugars and carbs too, well it can be tricky, but definitely doable. Everyone has already given you some great tips. Also, there are some older posts that give a lot of helpful information too. So please feel free to look around the team posts and jump right in with any thoughts, ideas, questions, helpful hints, etc. when you are ready.
As for the coffee creamer. I know Nestle's Coffemate makes some sugar free coffee creamers that do not taste bad and they are not terribly high in sodium, maybe 5 mg or less....which is not that bad, as milk also has a certain level of sodium in it too. I am not sure if International Delights has any sugar free creamers or not. Other options, if you are wanting flavors to your coffee, is that Barney's Coffee and sometimes Starbucks sell some sugar free coffee flavorings.
