It's $10 a month, but the first 6 months is included in the purchase of the device. You also get discounts for buying in chunks of months at a time. You can wear it 24 hours a day if you really want to, but I take mine off at night, I sleep slightly better without it. Basically you tell it when you normally get up and go to bed, and it assumes you're sleeping or at least relaxing during those times, unless the data from your Bodybugg tells it otherwise. When you upload your data to the website, it asks you what you were doing while it wasn't on your arm, and you can manually put stuff in, e.g. showering, swimming (you can't get it wet) or just inactive. It straps onto your left arm, sitting on your tricep.
If you're considering replacing your HRM with it, just remember that it doesn't track heart rate at all, because it doesn't consider heart rate a good indicator of calorie burn. If you want to be able to track your heart rate, you'll still need some kind of HRM.
Having said all that, my bodybugg is broken today :( It won't turn on. The three others we have in the family are all fine, so I'm not too worried about reliability, just sad that I won't have it there to rely on for the next few days while they send me a new one. They also gave me two extra months subscription free for the inconvenience, so I really can't complain at all. I knew I relied on it, but never realised -quite- how much until I'm now stuck without it for a bit!
