It may also be bad posture caused from stiff muscles. Are you stretching well after working out? If not, your muscles may be stiff and you may be slouching more to compensate without even realizing it. Even if you are stretching, they may still get stiff for awhile, so just be aware of your posture throughout the day

I agree with the post below... I am doing my thesis and working right now, which has me sitting at a computer for 8 to 10 hours a day, and I know that if I don't make a conscious effort to maintain good posture, or if I'm not in an ergonomic chair, my upper back and shoulders start to absolutely kill by 5pm. Or perhaps you've been more stressed lately? Lots of people carry their stress in their upper backs and shoulders, by tensing up, hunching forward, etc. It could be related to that too.
If this is the case and your pain is related to poor posture, I like to keep a post-it note on my computer that just says, "sit up straight! No slouching!" and one that reminds me to get up and walk around every hour or so.

Because we don't know anything about you or your workouts, we can't tell you what you're doing wrong.
If you feel pain between your shoulder blades, it sounds like you have a strain or muscle pull of some sort. But, that's just a guess. If you recently started working out, it may be you did a little too much for your body to handle even though it might not have seemed like too much initially.
It sounds like you resumed working out before your muscles had properly healed. I would suggest laying off any exercise that works that part of your body. I would also suggest talking to a personal trainer or group exercise instructor at a local gym. Have them look at your weight training exercises for form.
Your back problem could be a result of doing certain weight training exercises with poor form. When lifting a weight, it is important to lift with good form or we do risk an injury of this kind.
These are a few thoughts, we really need more information from you. Could you tell us a bit more about your weight training routine ? What exercises do you do for your back and shoulders ? What weight are you using ? Where did you learn your form ? What PT or DVD or youtube video ?
For the past 2 weeks I've had this achiness in my upper back between my shoulder blades. I work out every day and I have tried to take it easy on the upper body stuff, but each day around 5pm my back feels tired and a little achey again. I took off 4 days from working out because I was out of town. My back got all better and I felt like the tiredness was over. I went back to my routine on Monday - cardio for 15 minutes, weights, and cardio for another 10 minutes - and I'm a little sore today but everything went fine. But the tiredness between my shoulder blades is back again. What could it be? The bench press? The overhead press? I don't know how to lay off it because it doesn't bother me while I work out and I don't know what's causing it.
