I struggled for a while with adult acne until I switched to a new soap. I read an article that mentioned African Black Soap, so I decided to give it a try and bought one bar. Now it's the only soap I use. The brand that I buy is Dudu Osun and I get it from amazon (they have the best prices).
I get it, too. Products with glycolic acids as the active ingredient have been most helpful for me versus the usual salicylic acid that many anti-acne products have. I don't wear heavy makeup to work out (and usually try to clean my face before a workout) and then wipe my face/neck down with a cleaning towelette as soon as possible. I also see an aesthetician once a month - an expense, yes, but my acne is the closest to under control as it has been in my life, even better than when I was visiting a dermatologist regularly.
I'm about to turn 50 and I still have acne issues. (talk about unfair!) Are you showering right away? If I don't get my sports bra off and shower immediately, I start to break out. Also, you aren't wearing make-up while you work out, are you? Otherwise, I would suggest upping your water intake, which usually helps flush stuff out of your system, and trying a different soap. I have to use something with glycolic acid in it, or my skin breaks out.

It could even be the cream your dermo gave you. Not all active ingredients are the same and I react badly to one but not the other. If you have a perscription and you're not seeing improvement in 2-4 weeks, go back to your doctor.

I have had the same issue and even though it does increase with working out, I find that the best way to prevent/control it is actually diet.
Of course cleanliness is hugely important, but when I eat chocolate or anything very oily or an excess of fatty items, it causes a significant spike in acne issues. Combining cleaner eating (not usually fun :() with the showering that you are already mentioning helps alot.
On another note, alot of times when people have bad acne they are either not showering well (ie not using a washcloth for the face or loofa for the back) or are scrubbbing TOO hard which irritates the skin and causes more issues.
