You might want to consider starting with something like Tai Chi or perhaps Yoga. These forms of exercise can be gentler to the body for a beginner with some physical issues. They focus on stretching, strength, balance, etc. and might help you "make friends with your body" and prepare both your body and your mind for other forms of exercise later.

I agree with Zorbs - weight loss is 80% nutrition and just 20% exercicse. One of the best places to start is to track your food. You don't need to become obsessive about it - just a couple of weeks of tracking can be really educative about where your calories are coming from, and can help with portion control and identifying some areas where you feel you can kokc off a few hundred calories without feeling deprived.

First, get medical clearance to exercise from your doctor. Then try walking for 1-5 minutes several times a day. Water exercise is good too if you have access to a pool. Also remember that weight loss comes mostly from exercise, so if your eating habits are as good as you say, you should be already seeing some weight loss results.
