What supports do I need to meet short and medium range goals?
Are those supports in place?
Time and money come to mind. Time to shop for and prepare healthy food as well as time to exercise. Money for healthy foods, good exercise shoes, perhaps some other equipment or a gym membership or trainer. An encouraging partner is a big bonus.
But, really, it takes very little beyond my own desire and integrity to get moving in the right direction. Interference mostly comes from my own excuses. Like many others here, I have health issues that can be challenging, but progress is still progress regardless of how slow it might be. Life is good. Hope you have what you need to meet all your goals.
