Why don’t people trust the ocean?

In October 1984, filming was under way on the seventh episode of the CBS spy series Cover Up. A scene called for actor Jon-Erik Hexum, as CIA agent Mac Harper, to load a gun. Filming was delayed, and Hexum got bored, so he started playing Russian roulette with what he thought was a harmless prop gun. But at close range, even blanks can be deadly. Hexum placed the gun to his temple, pulled the trigger, and a blank discharged into his brain. He died six days later. Production on Cover Up continued—a new character was substituted for the rest of the season. (And Hexum’s absence was left unexplained.)
Longest-running TV series of any kind: Meet the Press, airing on NBC Sunday mornings since 1947.
