The American Heart Association has been publishing infographics on how to prevent heart disease. The series called "Life's Simple 7" features a different aspect of life - diet, cholesterol control, smoking, etc.
I like the infographics easy to read, right to the point approach. You can print them out and keep posted where you can see them on a daily basis. They offer some tools to help you achieve your goals.
