It helps me to find some kind of motivation--a small reward after a certain number of points or amount of exercise, using my favorite lotion after exercise, etc. Find what works for you. If your family is motivation for you to be healthy, maybe post extra pictures of them in places where you need help--on the fridge, on the TV, etc.
