Starting Over For the Last Time!

I am so sick of starting over! I would like to get it right once and for all. I do good for a couple weeks, then go back to my old ways. And I feel like I start over every week. The weekends are killer! I am great at maintaining, just not losing. I wish I could afford a Personal Trainer to kick my butt into gear. But I need to realize that this has to be me, no one else can do it for me. So here are my reasons for sticking to my goals and losing weight:
I hate the thought of things getting the best of me! Obesity in my family is not gonna keep me down (so to speak)
I want to feel in control, not controlled by cravings or emotional eating
I want to avoid family health problems (Diabetes, Obesity, High Blood Pressure, etc.)
I want to feel good physically-not constantly tired and lethargic
I want to feel good about myself, not ashamed to wear certain clothes or do certain activities
I want to be more athletic and active. When there's something I want to do, I don't want my weight or the fact I'm "out of shape" hold me back.
I want to enjoy clothes shopping
I don't want to be embarrassed in a bathing suit
I want to feel good about me
