Tired of the scale reading higher and higher

Hi, I'm 49 and a 4yr post gastric bypass patient. I lost 105lbs, but over the past 2 1/2 years I have regained 30 lbs. I want to get back on track and eliminate my bad habits to once again reach my goal weight.
