I sometimes feel a bit overwhelmed when trying to get back on track so I just try to pick one thing( like meal planning 3 days at a time). After a week I start to feel more confident and in control so I add 10 - 15 minutes of exercise 3 days a week. It may seem trivial after what I was used to but it is a heck of a lot better than doing nothing. Little by little the motivation is returning and the scale is showing results. Just start slow and take it a day at a time. You'll get there. I also joined the 30-day small changes challenge. Challenges help keep me accountable.
