I do not know much about gout, but are you able to do arm exercises with weights while sitting, or even simple arm circles? That and concentrating on diet (make sure you eat enough! Six meals a day if possible!) might help. Good Luck!
Gout is the worst, so I sympathize with you! I had an outbreak of it a few years ago, and I am in none of the risk categories for it. The pain was so bad, I actually accepted a ride from a strange man as I limped home from the store (who was the genius who wanted to live in the "Hilltop" neighborhood???). Luckily, fresh cherries are almost in season, and you can snack on those to help ease inflammation. In the meantime, concentrate on your diet. That's the big key to preventing gout and losing weight.

Don't worry !! Unless you ate an extra 14,000 calories in the past couple of weeks, you did not gain four pounds of fat. In order to truly gain one pound of fat, a person needs to eat an extra 3,500 calories on top of their normal intake. So, if you know you didn't eat that much extra food, then you know you didn't gain fat.
Why did you gain weight ? You're probably retaining water. Ever notice your weight goes up during TOM ? Most women tend to gain weight during their menstrual cycle. Is that a fat gain ? Nope, it's nothing more than a temporary water weight gain that passes in a few days.
I can easily gain or lose as much as 3-4 pounds in a day because of a shift in my water weight.
Try not to worry ! Do your best to eat right and watch your portions. I person CAN still lose weight as long as they eat right to the best of their ability.

I gained 4 more pounds. I'm not doing too good. I still can't exercise from having Gout in my ankle. I'll lose it somehow. I know it! I weigh 215 pounds now.
