Proper shoes, and listening to your body.
I did step aerobics for almost a year, and had no trouble. You DO have to watch your placement, and move at YOUR pace... not the instructor's pace. You have to remember the people who lead thse classes are fit, and do it constantly, and they're choosing where to go! I often stay a step or two behind so that I ensure my foot lands properly.
I have a weak ankle (leftover sprain from a few years ago) but never had trouble, because I was always careful, and wore non-slip shoes. Having the proper height on your step is important, too. Some of our less-able classmates would do theirs flat on the floor when they couldn't do all the steps.
Step aerobics is like any exercise. You have to use good form, be careful, and listen to your body!

If you keep the step low, you should be fine. Step is actually low impact cardio unless you take it to another level like jumping over it..
I do the step machine often, and have not had any problems. I make sure to stretch very well to avoid problems. Especially Hip Flexors, Quads, and ITB.

I agree that you just need to pay attention to what you're doing, as any exercise. I have heard of a few people getting hurt at my gym but that's seriously a few in over 6 years with several classes happening on a daily basis that may use a step.
Some pointers: always make sure you get your entire foot on the step. And you don't need to stomp on it; we shouldn't hear you coming! . Also, make sure the step is either skid proof and/or on a none overly slippery surface. I think most steps have rubber edges on them which help keep them still.

I have one and all you have to do is learn to be careful.
Getting balance at first can be tricky but after a few day it is like crossing the road.

no knee injuries, but I had shin splints, twisted ankles galore, plantar fascitis that all disappeared when I started running instead.
