the only major thing I would adjust on your list is put Get Healthy first. Do that and weight loss, less carbs etc will certainly follow

Plan for the year - drink more water, eat less carbs, lose wt, get healthy, and exercise more. Ok so lets get specific- 8-10 cups water per day, less than 50 gms carbs, lose 25 lbs, go to exercise 2-5 times / week.
