you're going to have to ask at the place. and if that diner has a place that makes bread for them as opposed to distributes bread to them, it's going to be hard because that bread won't have labels on it. if they do get distributor bread, they may be more inclined to switch between similar items which are on sale. i work in a situation where i am in an out of various kitchens throughout the week and there simply isn't a universal bread set option. each place varies depending on what its customers prefer and what its vendors provide. so you have to ask what they have available and then decide from that set.

I ask them to hold the bread, I order a side salad and put the sandwich components on the the salad. Either that or I just eat the components by themselves with a knife and fork. Good stuff. :)

Good question...definitely the rye or sourdough if what you suspect about the 'whole wheat' is true. They have a much lower glycemic index than the white bread.
Being in NJ, one of the usual spots to get dinner after youth group is a local diner (there are a few we frequent). While, at times, I may indulge and get a grilled cheese sandwich, I figure a turkey sandwich (turkey, lettuce, onion, mustard) with a side of fruit is a better option.
I'm just not sure what bread is safest. I know most diners have wheat, white, and rye breads, maybe sourdough... and there's a good chance it's NOT 100% whole wheat bread, but brown colored white bread instead.
