Couple of responses for you and you can take them or leave them.
Don't have chocolate in the house. Your child will learn that it is not there and is not a snack food (just a rare treat for now and then).
I know it's hard to keep them still (I have a 7 and 5 year old), but even when they were young, we did have dinner at the table. At first, the sitting time was pretty short, but they asked to be excused before leaving the table (all this is habit now and I'd suggest not waiting too long to start as it will get harder). At an early age you can start with a few basic table manners and go from there.
