Because you have a heart problem, you need to talk with your doctor to find out the best things for you to do. Your doctor knows and understands your condition and circumstances and is the only one that can truly give you the best information to help you lose weight in a safe way for your condition.
In general though, eating healthy, nutritious foods and getting exercise (cardio & strength training) will help you reach a healthy weight and body.
I would recommend that you take a look at these articles to help you get started in the right direction:

But you should definitely be talking to your doctor!

I would recommend something low impact, such as walking or swimming when you first start out. When you start to lose a little weight, than things such as jogging, or biking become optional. Its an uphill battle, and I know it from experience it can be hard.
